The Essential Features of the YTP have sustained since the program's inception in 1990. Upload the 2 attached essential feature documents below (Essential Features #1 and #2) and consider these features as the foundation of best practice.

Program Management Features are also extremely important as YTP sites build their partnerships with OVRS Branch Offices. Those features include:

1. YTP Staff Roles: YTP staff must have clearly developed roles and responsibilities, which make use of individual team member's strengths and interests.

2. Schools/OVRS Collaboration: YTP school and OVRS staff meet regularly to review client progress and coordinate delivery of services.

3. Managing Paperwork: YTP staff must develop a system for managing student and program paperwork in order to evaluate student progress, communicate with their OVRS partner, and document services provided.

4. Coordination with Other Programs: A process must be developed for coordinating the YTP with existing school and community programs in the delivery of transition services.

5. Marketing and Public Relations: YTP staff must create opportunities for marketing the program in order to more effectively integrate YTP services into the school and community.

COMMUNITY MAPPING

Best practice in transition planning also includes Community Mapping. For a great resource on this topic, go to: http://ncset.org/publications/essentialtools/mapping/default.asp

You will be able to download a *.pdf document (look in the upper right hand corner of the page) or sections of a *.pdf document on the topic of Community Mapping.

SHARING YTP CLIENT PROGRESS NOTES WITH VOCATIONAL REHABILITATION COUNSELORS ON A REGULAR BASIS

As a best practice, YTP Transition Specialists should share YTP client progress notes electronically with their Vocational Rehabilitation Counselor (VRC) partners on a systematic and scheduled basis. Every 2 months would be a good schedule to consider.

The electronic (e.g. email, progress notes in document form attached to an email, etc.) sharing of progress notes allows the VRC to copy and paste those notes into their ORCA system (OVRS's data management system). It also fosters communication, teamwork, collaboration, "next step" thinking in terms of plan development, readiness for employment, and other benchmarks along the path of rehabilitation that VRCs are required to report on.
